So you’re inserting 10%+ of your database size every day? That seems weird to me. Are you pruning somewhere? If not, is your database less than a month old? I’m confused.

Well, that's why I said "ATM", it's not a sustained rate, all the time. And yes, there's a bunch of DELETEs too.
